Your Visit
During your initial consultation, you will meet with Dr. Hasen and our patient coordinator to discuss your concerns and desires. We will treat you with respect, dignity and understanding during the entire process. We will strive to provide you with outstanding patient education which helps you choose the best procedure for yourself. This will provide you with an understanding of the surgery and the recovery process. You will receive a customized information packet covering the procedure(s) you may be considering. As we are in full compliance with HIPAA regulations, you can be assured that we will hold all information that you share strictly confidential.

To save time before your initial consultation, please download our registration forms and fill them out. Please call our office if you would like these forms mailed or faxed to you instead. If you complete the forms and send them back via fax (239) 262-5639 or mail to our office at 4081 Tamiami Trail North, Suite C203, Naples, FL 34103, we will have your paper work completed when you arrive. You should plan to spend about 1 1/2 hours with us, allowing enough time to meet with Dr. Hasen, to take digital photos, to perform computer morphing imaging (if appropriate) and to speak with the patient coordinator about pricing and scheduling.
