Your Visit
During your initial consultation, you will meet with Dr. Hasen and our patient coordinator to discuss your concerns and desires. We will treat you with respect, dignity and understanding during the entire process. We will strive to provide you with outstanding patient education which helps you choose the best procedure for yourself. This will provide you with an understanding of the surgery and the recovery process. You will receive a customized information packet covering the procedure(s) you may be considering. As we are in full compliance with HIPAA regulations, you can be assured that we will hold all information that you share strictly confidential.

To save time before your initial consultation, we have implemented a web based patient registration process that is easy, secure and efficient. You should have received an access code number from our staff to allow you to login to this secure site. Please call our office if you need an access code number and we would be happy to supply you with one. You should plan to arrive 15 minutes before your scheduled initial consultation to confirm your paperwork is complete and sign the forms that we cannot place on the internet.
Returning patients will also be asked to update their information on a yearly basis. Our staff will supply you with an access code before your next visit to our office.
