Thermage® works by delivering radiofrequency (RF) energy deep into the skin to create a uniform and volumetric heating effect in the mid and deep layers of the skin. There is an internal cooling spray in the treatment tip that cools and protects the surface of the skin. You will feel and hear this cooling spray when the treatment tip is contacting your skin.
A reverse thermal gradient is created in the skin with the Thermage® procedure. An internal contact coolant within the treatment tip is used to cool and protect the top layer of skin while simultaneously heating the mid and deep layers of the skin. This temperature rise in the deeper layers of the skin is designed to cause immediate contraction of collagen fibers and gradual collagen tightening and production of new collagen over time. During treatment, you may experience a sensation of deep heat.

What is the "Face by Thermage™" procedure?
Thermage® is a name for the skin tightening procedure that is performed by Kent V. Hasen, M.D. using the ThermaCool TC® radiofrequency system. This procedure allows him to treat aging and sun-damaged skin by delivering deep intense heat without harming the top layer of the skin, the epidermis.
How does Thermage® work?
The ThermaCool TC® device delivers safe radiofrequency energy that penetrates the top layer of the skin and creates a volumetric and uniform heating effect to the deep dermis. Thermage® reduces wrinkles and achieves skin lifting by skin tissue tightening caused by immediate collagen contraction followed by gradual collagen tightening. The result is a more youthful appearance to lax and sagging skin.
Where on the face is "Face by Thermage™" performed?
Even a small amount of skin tightening in certain key areas can be advantageous for significant cosmetic improvement. For example treating the skin of the forehead can lift the eyebrows and enhance the appearance of the eyes, making the eyes appear more open, alert and less tired. Previously, this result could only be obtained with major surgery. In June of 2004, the FDA approved the use of the ThermaCool TC® device for tightening the skin of the face, jowls and neck.

Is Thermage® painful?
Each time the ThermaCool TC® device is pulsed there is a brief sensation of intense heat in the skin. The ThermaCool TC® device monitors the surface temperature of the skin and applies a cooling cryogen spray, which is delivered before, during and after the application of heat. Additionally, Dr. Hasen prescribes a pain pill and/or a relaxing pill to take before the Thermage® procedure.
Is the Thermage® procedure using the ThermaCool TC® device safe?
Yes, the ThermaCool TC® device that is used in the Thermage procedure has been FDA approved for the reduction of wrinkles and rhytids of the face, neck and brow as well as all over the rest of the body. Complications are extremely rare. Some tingling sensation may occur along the jaw line for a few days. There may be some minor swelling for the first two to three days. You can usually return to your daily activity the next day.
How does my skin change?
On the surface, the texture and tone of your skin will appear more youthful. By four months after the Thermage® treatment, there is new collagen production in the deep layer of your skin called the dermis. This new collagen layer has been shown in studies to be thicker and improved from the original skin collagen in test patients.
How long does the Thermage® procedure take?
Usually, treatments take between one to two hours, depending upon the size of the treatment area.
Is there any special preparation or follow-up regime?
No. Unlike major invasive surgery, there is no need to interrupt any medications or to do any fasting the day before the Thermage® treatment. You can return to normal activities almost immediately. Post-treatment sunscreen protection with UVA/UVB coverage is necessary for at least two weeks.
Can Thermage® be performed on tanned skin?
Yes. Unlike light sources like IPL and certain lasers, Thermage® can be done on tanned and dark skin. The radio-frequency waves do not target the melanin containing pigment cells of the skin.
When will treatment results be noticed?
You may see some immediate change from the Thermage® procedure due to initial collagen contraction. Long term improvement generally occurs within three to six months. In clinical studies, 80% of the patients demonstrated significant improvement of wrinkle severity and tightening of the skin up to 6 months after a single treatment. In certain cases, a second treatment three to six months after the first may be warranted to get additional lifting and tightening of certain areas.

Who is a Candidate?
- Patients with loose and sagging skin of the brow, upper eyelid, face and neck.
- Patients with jowls and deep creases between the base of the nose and the corners of the mouth.
- Patients with a double chin or turkey neck.
- Patients with sunken cheek pads and a tired appearance.
Intended Result:
- A more rested and youthful contour to the face and neck without surgery or prolonged recovery.

Procedure Description:
- The FDA approved Thermage procedure is performed by Dr. Hasen in his office on an outpatient basis under oral sedation.
- No scalpels, incisions or cutting are needed to achieve this skin tightening.
- The ThermaCool TC Device® delivers non-laser radio-frequency wave energy to the deep layer of the skin where the structural protein of the skin -Collagen- is located.
- The top layer of skin (epidermis) is protected from the heat by a cryogen (cooling) spray to prevent burning.
Recuperation and Healing:
- The patient goes home with no dressing or bandages. Icing may help reduce the minimal swelling.
- Minimal initial discomfort is easily controlled with Tylenol.
- Swelling and small bruising may occur for two to three days at the sites of local injection around the eyes. Icing for the first night usually reduces this to a minimum.
- The tightening and lifting of the skin is gradual. The final result is evident in four to six months.
- At three months, an evaluation is made to see if a second treatment may be beneficial.
Additional Options:
- Additional procedures that would enhance the result are: BOTOX® Cosmetic and facial fillers like JUVÉDERM®, Restylane®, Perlane® and Radiesse®.

- This procedure is cosmetic and therefore is not covered by insurance. The patient is responsible for payment.
